The application below is good to apply for our Board of Directors, Committees, and become a volunteer. Has three sections. The first will ask you about yourself, the
second about your dog (s) if you have one (or more), and the third will tell us to which committee (s) would you like to belong ( you may apply to more than one), and
will let us know if you would like to be part of the Board of Directors, or be a volunteer (it is not mandatory to be part of committee to become a volunteer or vice
verse).

You will be asked questions like; Have you ever been convicted or charged for animal cruelty? or, Does your dog have a rabies license from Miami Dade County? It is
important for us in order to approve your application to know if you have a history of animal cruelty, and as a Responsible Dog Owners of Miami Beach, we need to
comply with the Miami Dade, and Miami Beach's laws and regulations.

The third part will ask you about the Committee (s), Board of Directors, or Volunteer you are interested on. You will receive an e-mail from that committee (s), Board
of Directors, Volunteer you are applying for within a week of your application's submission, inviting you for an orientation BB Que on April 30, on Hibiscus Island (it will
be a lot of fun). An R.D.Os. T-Shirt will be given to you, when the application process is complete.

After you fill each section, you will find a 'Submit' button, please click on it. A confirmation page will tell you that this particular part has been submitted, when this
happens, click on the back button, and do the same with the following part.

The due date to submit your application for the Board of Directors has been change to May, Sunday 7, for the Committees is May, Sunday 20, and for volunteer is
open the whole year.

Important: every field is mandatory, and must be filled. If does not apply to your situation, please write down NONE. To fill the complete application should take 10
minutes.Thank you.
